Job summary

Clinton Counseling Center in Mt. Clemens, MI seeks a Case Manager/Supports Coordinator on a contractual, part-time basis to support individuals in an outpatient clinic. Duties include assessment, IPOS planning, resource linkage, and advocacy within community services.

The role involves coordinating with other care providers, crisis intervention, and discharge planning, with supervision from licensed professionals and local travel required.

Qualifications

• Bachelor's degree in Social Work, Counseling, or Psychology is required.
• Master's degree preferred (LMSW/LPC or equivalent).
• CADC/CAADC preferred; MCBAP development plan required at time of hire.
• Experience is preferred but not mandatory.
• Valid driver's license with own vehicle and ability to pass a background check.
• Supervision provided by a fully licensed LPC or LMSW.

Responsibilities

• Assessment and reassessment of clients assigned to a caseload.
• Planning and/or facilitating planning using person-centered principles.
• Developing an IPOS using the person-centered planning process.
• Reviewing and updating the IPOS at least annually or whenever the client wants to review and update.
• Linking to, coordinating with, follow-up, advocacy with other mental health services and community services/supports.
• Brokering with providers of services/supports.
• Assistance with access to entitlements and/or legal representation.
• Coordination with the other care providers.
• Document clinical information in a complete and timely manner, according to accepted practices.
• Participate in case reviews as needed.
• Monitoring of services provided to the client.
• Crisis intervention.
• Discharge planning.
• Outreach/networking with other service providers as needed

Education

Bachelor's Degree (Social Work, Counseling or Psychology)

Master's Degree preferred (L/LMSW, L/LPC, T/LLP)

CADC/CAADC preferred, or MCBAP Development Plan required at time of hire

Valid Driver's License with own vehicle

Clinton Counseling Center is a Private, Non-Profit Outpatient Substance Abuse & Mental Health Clinic located in Mt. Clemens, MI. The Case Manger provides support to individuals receiving services through our outpatient clinic. This position is contractual, with flexible hours and some local travel in the community. The role of the case manager is to provide support, connection to community resources, and advocacy for persons served.
