Qualifications

• Bachelor’s degree in a human services field from an accredited institution.
• Valid Michigan Driver’s License.
• Willingness to work in the community and perform home visits.

Responsibilities

• Work in the community performing home visits.
• Coordinate all services to maximize consumer recovery and independence.
• Provide clinical and program consultation to direct care staff.
• Ensure case management compliance with agency standards and practice model.
• Assist consumers to improve quality of life within the community.
