Job summary

Community Empowerment Resources (CER) in Hawaii is seeking a compassionate Community-Based Case Manager to support adults living with serious mental illness in the community. You will develop recovery plans, coordinate services, and build trusting relationships to promote independence and stable living.

CER emphasizes person-centered, trauma-informed care with low caseloads of approximately 12–16 clients. You will collaborate with families, providers, and community partners to deliver

Qualifications

• Requires valid driver’s license.
• Reliable transportation for field-based client visits.
• Combination of education and/or experience with vulnerable populations to meet contract requirements.

Responsibilities

• Develop, implement, and update individualized service and recovery plans based on client goals, strengths, needs, and preferences.
• Build and maintain trusting, therapeutic, and recovery-oriented relationships to promote engagement, stability, and independence.
• Coordinate and facilitate access to psychiatric, medical, behavioral health, housing, legal, vocational, substance use, and other community-based services.
• Provide consistent, person-centered support through home visits and community-based meetings to address client needs and progress toward recovery goals.
• Collaborate with treatment providers, families, and community partners to coordinate care, address barriers, and promote continuity of services.
• Provide transportation and accompany clients to appointments or community resources when clinically appropriate and safe.
• Monitor client progress, identify emerging needs or barriers, and adjust service plans in collaboration with clients and the treatment team.
• Maintain accurate, timely, and thorough documentation in accordance with agency policies, confidentiality standards, and funding requirements.

Education

Bachelor's degree preferred in social work, psychology, counseling, nursing, or related field
