Job summary

Community Empowerment Resources (CER) seeks a compassionate Community-Based Case Manager to support adults with serious mental illness in the Honolulu area. You will build clinically informed relationships, coordinate services, and help clients move toward recovery with individualized plans.

CER emphasizes person-centered care, trauma-informed practices, and collaborative work with providers, families, and partners. Caseloads are intentionally low to ensure quality, responsive support.

Qualifications

• Valid driver's license.
• Reliable transportation for field-based client visits.
• A combination of education and/or experience with vulnerable populations to meet the minimum contract requirements.
• Bachelor's degree in social work, psychology, counseling, nursing, or a related field.
• 6–12 months of experience supporting individuals with SPMI or other special-needs populations.
• Experience with community-based case coordination.
• Experience maintaining accurate and timely case documentation and managing time effectively across community-based case management responsibilities.
• Experience collaborating with clinical and community partners.

Responsibilities

• Develop, implement, and update individualized service and recovery plans based on each client’s goals, strengths, needs, and preferences.
• Build trusting, therapeutic, and recovery-oriented relationships with clients that promote engagement, stability, and independence.
• Coordinate and facilitate access to psychiatric, medical, behavioral health, housing, legal, vocational, substance use, and other community-based services.
• Provide consistent, person-centered support through home visits and community-based meetings to address client needs and support progress toward recovery goals.
• Collaborate with treatment providers, families, and community partners to coordinate care, address barriers, and promote continuity of services.
• Provide transportation and accompany clients to appointments or community resources when clinically appropriate and safe.
• Monitor client progress, identify emerging needs or barriers, and adjust service plans in collaboration with clients and the treatment team.
• Maintain accurate, timely, and thorough documentation, including case notes, service plans, and progress updates, in accordance with agency policies, confidentiality standards, and funding requirements.
