Job summary

Community Empowerment Resources (CER) is seeking a compassionate Community-Based Case Manager to provide individualized support in the community for adults living with serious mental illness. You will build trusting relationships, coordinate services across behavioral health, housing, medical, and social needs, and help clients work toward recovery and independence.

Qualifications

• Must have a valid driver's license.
• Reliable transportation for field-based visits.
• Experience working with vulnerable populations to meet program requirements.

Responsibilities

• Develop, implement, and update individualized service and recovery plans based on client goals, strengths, needs and preferences.
• Build and maintain trusting, therapeutic, recovery-oriented relationships with clients.
• Coordinate access to psychiatric, medical, behavioral health, housing, legal, vocational, substance use and other services.
• Provide home visits and community-based meetings to support recovery goals.
• Collaborate with treatment providers, families, and community partners to coordinate care and address barriers.
• Provide transportation and accompany clients to appointments when appropriate and safe.
• Monitor progress, identify needs or barriers, and adjust plans with clients and the treatment team.
• Maintain accurate documentation per agency policies and funding requirements.

Skills
