Job summary

Merakey is seeking a community-based Blended Case Manager in Colmar, PA, serving Montgomery County. The role involves developing, coordinating, and implementing services for clients aged 6 to adult, with case management, service planning, and coordination with other providers to meet consumer needs and improve community stability.

The position includes home visits, community-based services, mileage reimbursement, and a typical schedule Monday–Friday 9:00am–5:00pm with optional on-call.

Qualifications

Valid driver’s license and clean MVR if required.
Basic computer skills and EMR experience preferred.

Responsibilities

Developing, coordinating, and implementing services for consumers as identified in the goal-oriented service plan.
Provide case management, service planning, and coordination with other agency providers.
Travel to homes and community locations to meet clients and support independence.
