Join Our Team as a Community-Based Blended Case Manager in Warminster, PA, serving Clients in Lower Bucks County!

As a Case Manager, you'll work directly in the community to empower individuals on their journey toward stability and independence.
In this role, you will:
- Build meaningful connections while assessing client needs and creating personalized care plans.
- Link clients to vital resources and advocate for their success.
- Deliver culturally responsive, strengths-based support that improves quality of life.
- Promote a rehabilitation and recovery-focused, consumer-driven, strength-based approach to case management and crisis services.
- Serve as primary case manager for assigned consumers, ensuring bi-weekly support contacts in appropriate settings.
- Develop and maintain consumer-driven service and safety plans, coordinate benefits, housing, and all aspects of service delivery.
- Locate consumers through family or community contacts when necessary.
- Collaborate with service providers, monitor progress, and maintain accurate records of hospitalizations, crises, and incarcerations.
- Gather and update social history and clinical summaries annually for each consumer.
- Participate in treatment team, service plan, and provider meetings; follow through on clinical decisions using problem-solving skills.
- Assist consumers with basic living needs, including housing, food, healthcare, transportation, socialization, education, and employment.
- Ensure annual physicals and monitor medical health through documentation.
- Complete reauthorization paperwork promptly and attend all meetings, supervision, and training professionally.
- Act as liaison with families, providers, and community supports; maintain professionalism per corporate policy.
- Complete all documentation accurately and on time; fulfill on-call duties as assigned.
This position is Community-based and requires travel to homes, and other community locations to provide services and support clients. Mileage reimbursement provided.

The ideal candidate will possess the following qualifications:
- A high school diploma plus a minimum of 12 college credit hours in Psychology, Sociology, Social Welfare, Gerontology required and 2 years of behavioral health direct care experience.
- A Bachelor's degree with major coursework in sociology, social work, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education.
The candidate will have all of the following:
- Must possess a valid driver's license, insurance and have a clean driving record.
- Own a reliable vehicle.
- Must be able to work 9am‑5pm Monday‑Friday – OPTIONAL PAID ON‑CALL AVAILABLE!
Skills preferred
- Experience in case management or related human services field preferred.
- Strong organizational and communication skills.
- Ability to work independently and manage a flexible schedule.
- Commitment to person-centered care and community-based service delivery.
