Job summary

Merakey is seeking a Blended Case Manager to support individuals and families in Lehigh and Northampton Counties. You will work in home and community settings, coordinating services, conducting outreach, and helping clients toward stability and wellbeing.

The role requires strong communication, problem-solving, and the ability to travel to client homes with mileage reimbursement. Pay starts at $20.07 per hour for full-time work, with an additional $2 per hour Enhanced Pay option.

Qualifications

• Case management or similar experience preferred.
• Education options include HS diploma with sociology credits or a BA in social science.
• Valid driver’s license and reliable vehicle for travel.

Responsibilities

• Serve as primary case manager with coordination of services.
• Conduct regular bi-weekly support contacts and plan development.
• Coordinate benefits, housing, and follow clinical decisions.
• Document social histories and update service-use summaries.
• Attend treatment and service plan meetings with families and providers.
