Job summary

Merakey is seeking a Blended Case Manager in Monaca, PA to support up to 30 individuals in the community by developing personalized service plans and coordinating with other providers. The role emphasizes hands-on case management and counseling to help clients reach their goals.

You will collaborate with county reentry grants and may participate in on-call coverage. A valid driver's license and OMHSAS clearances are required as part of the role.

Qualifications

• 2+ years’ experience in public or private human service, with 1 year direct care.
• Experience must be with clients over the age of 18.
• Valid Driver's license required.
• All clearances required by OMHSAS regulations.

Responsibilities

• Serve as the primary contact for up to 30 individuals in the community
• Develop, coordinate, and implement personalized service plans
• Provide case management, service planning, and coordination with other agency providers
• Offer supportive counseling to help individuals achieve their goals
• Work alongside county reentry grants to support individuals transitioning from jail back into the community
• Participate in on-call responsibilities as needed
