Job summary

Penndel Mental Health Center in Langhorne, PA is seeking a dedicated Blended Case Manager to help children, adolescents and adults access needed community services and supports to empower their recovery.

You will develop individual service plans, link clients to housing, education and health resources, coordinate with MH programs and providers, and maintain regular face‑to‑face and phone contacts, including after‑hours crisis calls.

Qualifications

• Bachelor's or RN or equivalent with related social services education.
• Two years of mental health direct contact experience if HS diploma.
• Experience in case management, outreach and linking clients to services.
• Ability to provide 24/7 on-call crisis support.

Responsibilities

• Assist chronic mentally ill clients in accessing services and basic needs.
• Link and monitor community service systems involved with the consumer.
• Develop and review individual service plans every 6 months.
• Maintain face-to-face and phone contacts with clients at least every 2 weeks.
• Coordinate with MH programs, group homes, hospitals, and providers.
• Provide housing resources and after-hours crisis support.
• Prepare daily documentation of BCM services.
• Attend trainings and supervision meetings.
• Adhere to Corporate Compliance policy.
