Job summary

Passavant Memorial Homes in Pennsylvania is seeking responsible ISO Support Service Workers to join its Home and Community Services program across Beaver County. You will provide care and supervision to individuals in daily living and community settings, using your own vehicle for transportation when appropriate.

The role emphasizes safety, positive role modeling, and adherence to IP plans, with training provided.

Qualifications

• Must be at least 18 years old.
• Valid driver's license required.
• Current car insurance and registration required (personal vehicle may be used).
• Successfully pass background check.
• Experience working with individuals with developmental disabilities preferred.

Responsibilities

• Provide care and supervision in activities of daily living and in social/recreational activities.
• Follow the individual's IP and perform assigned tasks.
• Ensure safety and serve as a positive role model in home and community.
• Complete pre-service and annual in-service trainings.
• Use the IP to guide services and report changes in condition.
