Job summary

Passavant Memorial Homes invites applications for ISO Support Service Worker positions in our Home and Community Services program. You will provide care and supervision in activities of daily living and in social/recreational activities, in the individual’s home or in the community, using your personal vehicle when appropriate.

The role emphasizes safety, positive role modeling, and adherence to our policies.

Qualifications

• 18+ years of age required.
• Valid driver’s license required.
• Current car insurance and registration required.
• Successfully pass background check.
• Experience with individuals with developmental disabilities preferred.

Responsibilities

• Perform tasks outlined in the individual’s IP.
• Complete assigned tasks as requested.
• Deliver services within IP-authorized units.
• Follow Passavant Memorial Homes policies and procedures.
• Provide supervision and positive role modeling at home and in the community.
• Complete required Pre-Service and Annual In-Service training.
• Observe behaviors and report changes accurately.
