Job summary
Meridian Behavioral Healthcare, Inc. is seeking a TIEH Team Lead to guide a multidisciplinary team serving individuals experiencing or at risk of homelessness.
You will supervise Care Coordinators, Clinician, Outreach Specialist, and Peer Support Specialist, collaborating across Meridian clinics and community settings to connect clients with housing, treatment, and recovery resources. Qualifications
- Master's degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or related field.
- Experience with homelessness or community outreach is preferred.
- Experience providing leadership in a behavioral health setting is preferred.
Responsibilities
- Lead an interdisciplinary team including Care Coordinators, Clinician, Outreach Specialist, and Peer Support Specialist.
- Oversee community outreach to engage homeless individuals and connect them with services.
- Build and maintain relationships with shelters, providers, and referral sources.
- Develop individualized service plans with clients to progress toward stable housing.
- Ensure accurate documentation and compliance with program requirements.
