Meridian Behavioral Healthcare, Inc. is seeking a TIEH Team Lead to guide a dedicated multidisciplinary team in supporting individuals experiencing homelessness.

The role involves community outreach, relationship building, and coordinating care to connect clients with essential services. Qualifications

• Master's degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or a related field.
• Experience working with individuals experiencing or at risk of homelessness is preferred.
• Experience serving individuals with mental health and/or substance use challenges is preferred.
• Previous experience in community outreach and care coordination is preferred.
• Previous supervisory, leadership, or management experience is preferred.

Responsibilities

• Lead and support an interdisciplinary team providing outreach and care coordination.
• Oversee community outreach efforts to identify and engage individuals eligible for services.
• Build and maintain strong relationships with community stakeholders and service providers.
• Monitor individual and team productivity toward program goals.
• Ensure timely and accurate client records and documentation.
