Job summary

Pacific Clinics will open a high‑intensity, community‑based Adult Full Service Partnership (FSP) program for adults 18+ with serious mental illness and complex needs. The program delivers “whatever‑it‑takes,” recovery‑oriented services primarily in the community to stabilize individuals, reduce hospitalizations and incarceration, prevent homelessness, and support long‑term recovery and community integration.

This program will also serve those involved with the justice system.

Qualifications

• High School Diploma or GED with four (4) years of relevant experience.
• CCAPP certification or drug & alcohol training required or experience.

Responsibilities

• Outreach and screening: participate in universal outreach activities to raise awareness of substance use among the target population.
• Substance use treatment: implement client intake, screening, assessment, and each client’s personalized plan.
• Recovery supports: assist in WRAP planning and recovery support efforts.
• Administration: collaborate with Program Manager, attend staff meetings, and coordinate with evaluation teams.
• Ensure follow-up interviews at six months post-discharge and complete outcome measures.
• All other duties as assigned.
