Aufgaben

• Maintain an active caseload of FSP participants requiring intensive, community-based support.
• Build trusting relationships by modeling recovery principles and empowerment.
• Collaborate with clinical staff, case managers, and psychiatrists for coordinated care.
• Provide outreach in the community including homes, shelters, and hospitals as needed.
• Help participants access housing resources, benefits, healthcare, and other supports.
• Support development of individualized recovery and wellness plans.
• Provide transportation and accompany to appointments, court dates, and housing meetings.
• Identify early relapse signs and help maintain stability.
• Facilitate peer support groups and wellness programming when appropriate.
• Document services complying with agency and program requirements.
