Job summary

Pacific Clinics will be opening a high-intensity, community-based Adult Full Service Partnership (FSP) program for adults 18+ with serious mental illness. The program delivers recovery-oriented services in the community to stabilize individuals, reduce hospitalizations, prevent homelessness, and support long-term recovery and community integration.

FSP ICM offers comprehensive services, either as a step-down from ACT or to avert ACT-level care for 144 individuals.

Qualifications

• High School Diploma or GED required, or equivalent combination of education and experience.

About Our Program

Pacific Clinics will be opening a high-intensity, community-based Adult Full Service Partnership (FSP) programs for adults age 18+ with serious mental illness (SMI) and complex needs. The program delivers “whatever‑it‑takes,” recovery-oriented services primarily in the community to stabilize individuals, reduce hospitalizations and incarceration, prevent homelessness, and support long‑term recovery and community integration.

FSP ICM offers a comprehensive array of community-based services and can be provided either as a step-down from ACT or as an intervention to avert the need for ACT-level care for 144 individuals. FSP ICM is delivered by a multidisciplinary team that incorporates core case management functions, such as assessment, planning, and linkage, with low staff-to-client ratios, assertive outreach, and direct service delivery. FSP ICM is for individuals who may not meet ACT eligibility criteria but still have significant behavioral health needs and can benefit from FSP supports.

Schedule

Monday - Friday 9 am - 6pm and as needed evenings, weekends and holidays, or to support a crisis

Pay Range

$21.00 - $25.83

Peer Partners play a vital role in promoting recovery, empowerment, and community connection for individuals in need of support.

Qualifications

• High School Diploma or GED required, or equivalent combination of education and experience (two years of relevant experience equals one year of education) to include: directly related experience as a recipient of behavioral health services, recovery services, or systems involvement (mental health, substance use, foster care, etc.) is required.

Level of Contract with Children

• May/will have unsupervised contact with children.
• Requires full background check and TB exam.
• Current First Aid/CPR Certification.
