Job summary

Beth Israel Lahey Health in Stratham, NH is seeking a Clinician (LICSW/LMHC) for the Primary Care Collaborative Care Program. Embedded in a primary care setting, you’ll work with a multidisciplinary team to identify, triage, and manage behavioral health and substance use concerns using brief therapies.

The role emphasizes collaboration with physicians, independent work, and flexible communication while delivering screenings, treatment planning, and education.

Qualifications

• Master’s degree in Social Work or related Human Service discipline.
• Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) required.

Responsibilities

• Review medical records prior to initial evaluation to familiarize with the patient’s history.
• Assess patients for behavioral health disorders and continuum of substance use disorders.
• Utilize brief treatment modalities (CBT, Motivational Interviewing, etc.).
• Design behavioral health treatment plans in collaboration with the primary care team.
• Develop and deliver behavioral health and substance use education curricula for staff.
• Complete clinical documentation per policies.
• Learn insurance standards for commercial and MassHealth products.
• Supervise and review performance with supervisor.
• Maintain professional relationships with physicians and the health care community.
