Qualifications

• Master's degree in Social Work or related field.

Responsibilities

• Review medical records prior to initial evaluation and accompany patient history review.
• Assess behavioral health and substance use disorders across the continuum.
• Apply brief therapies such as CBT, Motivational Interviewing, Problem-Solving Therapy, Behavioral Activation, psycho-education.
• Collaborate with primary care team to design treatment plans.
• Develop and deliver staff education curricula on behavioral health topics.
• Complete all clinical documentation within required timeframes.
• Maintain patient confidentiality and professional boundaries.
• Establish and maintain relationships with physicians, patients and external providers.
