Job summary

MFM Health in Beverly and Danvers, MA is seeking a Collaborative Care clinician (LICSW or LMHC) to support integrated behavioral health in a primary care setting.

You will engage patients, perform assessments, coordinate with primary care and behavioral health specialists, monitor progress, and document care in the electronic record. This full-time, in-office role emphasizes crisis support and evidence-based treatment planning.

Qualifications

• Master’s degree in Social Work, Therapy, Counseling, or Psychology.
• Active Massachusetts licensure as LICSW or LMHC required.
• Experience delivering behavioral health services in clinical settings.
• Experience with care coordination and interdisciplinary collaboration.
• Knowledge of Collaborative Care Model (CoCM) and primary care integration.
• Ability to document care accurately in electronic health records.

Responsibilities

• Engage patients in behavioral health care through education, support, and connection to resources.
• Conduct initial mental health assessments and screenings and evaluate co-occurring conditions.
• Coordinate communication, appointments, referrals, and follow-up among care providers.
• Develop individualized, evidence-based treatment plans with patient input.
• Monitor progress and coordinate treatment-plan adjustments with the care team.
• Provide crisis intervention support and coordinate emergency referrals when indicated.
• Maintain timely electronic health record documentation and support quality initiatives.
