Qualifications

• Master's degree in Social Work or other directly-related Human Service discipline.
• Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) required in NH.

Responsibilities

• Review medical records prior to initial evaluation to familiarize with patient history and perform thorough assessment.
• Assess patients for behavioral health disorders and the continuum of substance use disorders.
• Utilize brief treatment approaches such as CBT, Motivational Interviewing, Problem-Solving Therapy, Supportive Therapy, Behavioral Activation and Psycho-Education.
• Collaborate with the primary care team to design behavioral health treatment plans based on patient needs.
• Create and implement education curricula on behavioral health and substance use for staff and providers.
• Complete all clinical documentation within expected timeframes and per policies.
• Learn insurance standards for commercial and Mass Health products.
• Develop supervision plan and meet regularly with supervisor to discuss issues and performance.
• Build and maintain relationships with physicians, patients, visitors, external providers, and regulatory bodies.
• Maintain confidentiality and protect sensitive data at all times.
