Job summary

MFM Health in Beverly and Danvers is seeking a Collaborative Care clinician (LICSW or LMHC) to support integrated behavioral health in our primary care setting using CoCM. You will engage patients, conduct initial assessments, and coordinate care with the primary care team to develop individualized treatment plans.

The role emphasizes documentation in electronic health records, progress monitoring, crisis support, and collaboration with psychiatrists, therapists, and social workers.

Qualifications

• Master’s degree in Social Work, Therapy, Counseling, or Psychology required.
• Active Massachusetts LICSW or LMHC license required.
• Experience delivering outpatient behavioral health services and care coordination.

Responsibilities

• Engage patients in behavioral health care through education, support, and connection to appropriate resources.
• Conduct initial mental health assessments and screenings, including use of standardized assessment tools and evaluation of co-occurring conditions.
