Job summary

IVYPARK is seeking a Certified Nursing Assistant to provide routine daily nursing care per each resident's care plan in a licensed facility. You will verify resident identity, record vital information, and report changes to the Nurse Supervisor.

You will assist with daily living activities and ensure a safe, respectful environment. Responsibilities include observing residents, preparing for exams, and maintaining chart accuracy while following all policies.

Qualifications

• Minimum 10th grade education or equivalent.
• Certified Nursing Assistant license required.

Responsibilities

• Provide routine daily nursing care and services per care plan.
• Identify residents before treatments using wristband/photo ID.
• Report changes in resident condition to Nurse Supervisor/Charge Nurse.
• Record entries accurately on charts and flow sheets.
• Maintain confidentiality of protected health information.
• Assist with daily living activities and mobility as trained.
• Assist with medical tests preparation and post-care tasks.
• Observe and report injuries or changes in residents' condition.
• Follow established policies and procedures and staffing schedules.

Education
