Job summary

Heritage Trails Nursing and Rehabilitation Center is seeking a dedicated Certified Nursing Assistant to deliver routine daily nursing care to residents according to individualized care plans. You will verify resident identity before procedures, document observations, and promptly report any changes to the nurse on duty.

Qualifications

• Must be a licensed Certified Nursing Assistant in accordance with state laws.
• Ability to read, analyze and interpret general business documentation.
• Basic math and measurement skills.
• Ability to solve problems and respond to routine questions.
• CPR certification is preferred.

Responsibilities

• Provide routine daily nursing care to residents according to care plans.
• Identify residents using wristbands or photo IDs before procedures.
• Record observations clearly in charts and flow sheets.
• Report changes in residents' conditions promptly to the nurse on duty.
• Maintain confidentiality of resident information and follow HIPAA rules.
• Assist with transfers, transports and positioning of residents.
• Support residents with daily activities and personal care tasks.
• Follow policies and safety procedures for exposure to fluids and hazardous materials.
