Empath Health is seeking a compassionate Part-Time CNA/Program Aide to join our interdisciplinary PACE team in Pinellas Park, FL. You will assist participants with daily living, mobility, meals, and restorative care under supervision, contributing to a mission-driven care model.

Qualifications

• High School diploma or equivalent required.
• Current Certified Nursing Assistant (CNA) in the State of Florida.
• Continuing Education: As required for licensure.
• Home Health Aide certification with 75 hours completed preferred.
• Valid CPR certification from the American Red Cross or American Heart Association.

Responsibilities

• Consults daily with the Home Care Coordinator to update plan of care.
• Assists with personal hygiene (skin, mouth, hair, bathing).
• Assists with bed mobility and ambulation.
• Assists with prescribed exercises taught by staff.
• Prepares meals including therapeutic diets and assists with meals.
• Assists to bathroom or with commodes, bedpans, or urinals.
• Reports changes in condition to the Registered Nurse.
• Completes documentation of services provided.
• Maintains confidentiality of participant information.
• Participates in continuing education and training meetings.

Empath Health is seeking a compassionate Part-Time CNA/Program Aide to join our interdisciplinary PACE team in Pinellas Park, FL. Under the direction and supervision of the Day Center Supervisor, the CNA/Program Aide is responsible for providing and assisting participants with activities of daily living, restorative and supportive care. What You’ll Do

• Consults each day with the Home Care Coordinator (or designee) to be updated on participant’s status and any changes that need to be implemented on their plan of care
• Personal hygiene such as skin, mouth, hair care and bathing.
• Assists participants in and out of bed assisting with ambulation.
• Assisting with prescribed exercise which participants and aide have been taught by appropriate personnel.
• Prepares meals, including therapeutic diets, assists participants with meals.
• Assist participants to the bathroom or in using commodes, bedpans, or urinals.
• Reports changes in participant’s condition and needs to the Registered Nurse.
• Completes appropriate documentation of services provided
• Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
• Participates in continuing education classes and any required staff and training meetings.
