Pinnacle Home Care in Florida is hiring CNAs and Home Health Aides to provide compassionate, in-home care across the state. You’ll follow care plans, assist with daily living activities, and support patients and families in a patient-centered environment.

Qualifikationen

• Must be CNA or HHA licensed
• Must have completed at least 75 hours of training including personal care concepts
• Must be in good health with no conditions injurious to patient or coworkers
• Strong ability to understand personal care basics, housekeeping, and meal prep
• Valid driver’s license and access to insured vehicle
• Able to visit homes that may not be wheelchair accessible
• Sufficient hearing, vision and dexterity for care techniques
• Physically capable of walking, standing, bending, and patient handling

Aufgaben

• Adhere to care plan to ensure safe, competent patient care
• Assist with personal hygiene and maintain a clean, safe home environment
• Assist with ambulation as prescribed by clinician
• Support therapy staff during rehabilitation
• Encourage patient independence per nursing plan
• Engage patients to promote mental alertness
• Provide emotional support to patients and family; maintain confidentiality
• Report changes to RN or clinical manager
• Perform light housework as directed
• Complete visit reports promptly in EMR
• Confirm daily visit schedules for coordination
• Participate in QA/QI program and improvement processes
• Follow policy for patient rights and discharge procedures
