Job summary

Action Health Staffing seeks a Certified Nursing Assistant to provide home care services in clients’ residences on an every-other-weekend schedule. The role focuses on direct patient care, personal hygiene, transfers, and adherence to the Plan of Care, without medication administration. Excellent benefits accompany competitive pay.

The CNA will work under supervision, coordinate with therapists as needed, document care activities, and ensure safety and dignity for each client in a home setting.

Qualifications

• Maintain certification with DFS.
• High school diploma or GED preferred.
• Possess effective communication skills to include ability to read, write and follow directions.
• Ability to perform physical tasks necessary to treat clients who may include lifting, bending and standing.
• Must have sympathetic attitude toward the care of the sick.

Responsibilities

• Provide direct patient care as assigned.
• Provide personal hygiene care (bath, shave, hair care, mouth care).
• Assist with transfers, repositioning, and ambulation.
• Provide basic nursing care and prescribed exercises as instructed by supervising clinicians.
• Prepare meals with consideration of client preferences and diets.
• Provide companionship and activities tailored to client needs.
• Light housekeeping duties as assigned.
• Maintain complete and accurate records as required by agency.
• Record and report changes in client’s condition.
• Complete EVV plan tasks and timesheets as needed.
• Demonstrate dependability by punctual reporting.
• Adapt to client and family value systems.
• Assist in promoting independence within client abilities.
• Perform other duties per the care plan.
• Always follow the Plan of Care — Never administer medications.
