Job summary

Action Health Staffing is seeking a Certified Nursing Assistant to provide in-home care services to clients in the patient’s residence. The role emphasizes direct patient care, personal hygiene, mobility assistance, and support with daily living activities.

Responsibilities include charting per agency procedures, coordinating with supervising nurses and therapists, and ensuring safety and dignity in home settings.

Qualifications

• Maintain DFS certification as required.
• Excellent written and spoken communication skills.
• Ability to read, write, and follow directions accurately.
• Physical capability to assist with lifting, transfers, and mobility.

Responsibilities

• Provide direct patient care as assigned in the home.
• Assist with personal hygiene and activities of daily living.
• Support transfers, repositioning, and ambulation.
• Perform basic nursing tasks under supervision per plan of care.
• Prepare meals as directed and consider dietary needs.
• Provide companionship and engage in client-centered activities.
• Light housekeeping and upkeep of living areas as needed.
• Maintain accurate client records and EVV task logs.
• Report changes in client condition to supervising staff.
• Adhere to Plan of Care and do not administer medications.
