Job summary

Action Health Staffing seeks a Certified Nursing Assistant to provide home care in patients' residences, delivering direct patient care and support as part of a care team.

The role emphasizes following the Plan of Care, maintaining client safety, and supporting independence where possible; travel within the assigned area is expected. The candidate should have a DFS certification and be capable of lifting and assisting with daily living activities.

Qualifications

• Maintain DFS certification.
• High school diploma or GED preferred.
• Ability to read, write and follow directions.
• Ability to perform physical tasks such as lifting, bending and standing.
• Sympathetic attitude toward care of the sick.

Responsibilities

• Provide direct patient care as assigned.
• Assist with personal hygiene care (bathing, shaving, hair care, mouth care).
• Assist transfers, repositioning, and ambulation.
• Provide basic nursing care as instructed by supervising RN or therapists.
• Prepare meals considering client preferences and diets.
• Provide companionship and activities as per client needs.
