Job summary

Action Health Staffing is seeking a Certified Nursing Assistant to provide home-based care to clients in their residences, including weekend rotations. The role emphasizes compassionate, patient-centered support and adherence to individualized care plans.

You will perform direct care tasks, assist with daily living activities, monitor client condition, and document changes. The position involves light housekeeping, meal preparation, and ensuring safety and dignity in the home setting.

Qualifications

• Maintain certification with DFS.
• High school diploma or GED preferred.
• Possess effective communication skills to include ability to read, write and follow directions.
• Ability to perform physical tasks necessary to treat clients who may include lifting, bending and standing.
• Must have sympathetic attitude toward the care of the sick.

Responsibilities

• Provide direct patient care as assigned.
• Provide personal hygiene care and daily living activities.
• Assist with transfers, repositioning, and ambulation.
• Provide basic nursing care and prescribed exercises as instructed by supervising clinicians.
• Prepare meals considering client preferences and special diets when directed.
• Provide companionship and activities tailored to individual needs.
• Light housekeeping duties as assigned.
• Maintain complete and accurate records as required by agency.
• Record and report changes in client's condition.
• Complete tasks per EVV app and time sheets.
• Demonstrate dependability by punctuality.
• Adapt to client and family value systems.
• Assist in promoting independence within client abilities.
• Perform other duties per care plan.
• Always follow Plan of Care — Never administer medications.
