Job summary

Action Health Staffing seeks a compassionate Certified Nursing Assistant to provide in-home care to clients, with emphasis on every other weekend and flexible scheduling. You will assist with daily living activities, hygiene, mobility, and supportive exercises under RN supervision, while maintaining patient comfort and safety in the home environment.

Responsibilities include documenting care, coordinating with the care team, and ensuring adherence to the Plan of Care.

Qualifications

• Maintain DFS certification as required.
• High school diploma or GED preferred.
• Strong reading, writing, and comprehension.
• Able to perform physical tasks including lifting.
• Compassionate and sympathetic towards the sick.

Responsibilities

• Provide direct patient care as assigned.
• Assist with personal hygiene and daily living activities.
• Support transfers, repositioning and ambulation.
• Provide basic nursing care and prescribed exercises as directed by supervisors.
• Prepare meals per client dietary needs and preferences.
• Provide companionship and age-appropriate activities.
• Light housekeeping as needed and follow care plan.
• Record changes and maintain accurate documentation
