Job summary

Liberty Health and Rehab in Advance, NC (Bermuda Commons) is seeking a Transportation Coordinator (CNA/EMT) to manage all resident transports, ensure vehicle maintenance, and coordinate care with hospitals and clinics.

Qualified applicants will hold a valid NC driver’s license, be 21+, and currently registered as a Nursing Assistant or EMT in NC, with CPR certification. The role requires planning, organizing, and performing well under pressure in a compassionate, teamwork-focused environment.

Qualifications

• Must possess a valid NC Driver’s License and clean driving record.
• Must be 21+ and of good moral character with elderly/disabled care empathy.
• Must be registered with DFS as a Nursing Assistant or as an EMT in North Carolina.
• Current CPR certification from the American Heart Association.

Responsibilities

• Coordinate all aspects of transporting residents of the facility.
• Assure proper maintenance of all facility vehicles.
• Represent the facility in a professional manner when dealing with medical providers that patients are transported to.
• Make all appointments for patients and maintain a calendar of appointments.
• Maintain required logs of each transport and for each vehicle.
• Ensure routine maintenance is carried out on any vehicle driven to transport residents.
• Establish and maintains a maintenance schedule for each vehicle used in transports.
• Assist in maintaining good working relationships with the community, hospital, and other health care facilities and agencies and assure coordination of services for patients.
• Take specimens and other items to the hospital or medical clinic as requested.
• Make purchases from local suppliers as requested by facility staff.
