Job summary

Moments Hospice is seeking a CNA Coordinator to join our office team in Wisconsin. The role focuses on scheduling and mapping all CNA coverage, supporting the hospice care team with efficient administrative processes.

Ideal candidates will have outpatient scheduling experience, state CNA certifications, and comfort working with EMR systems such as HCHB. A valid driver’s license and reliable vehicle are required to fulfill field-related duties.

Qualifications

• HCHB EMR experience preferred.
• Outpatient Scheduling Experience required.
• Certified in state(s) practicing as a Nurse’s Aide or CHHA/CGNA; non-aide acceptable with admin approval.
• Board Certified CHPNA (Preferred).
• At least two years’ hospice or homecare experience.
• Must have valid driver’s license and auto insurance.
• One year data entry/medical records experience in healthcare customer service.
• Preferred graduate of an accredited LPN or Vocational Nursing program.
• Knowledge of Home Care Home Base EMR system.

Overview

Moments Hospice recognizes the important role of the Hospice Aide on the hospice team. The CNA Coordinator is a member of the office administrative staff and assists in other tasks as needed. The CNA coordinator will be responsible for the schedule and mapping of all CNA schedules.

Qualifications

• HCHB EMR experience preferred
• Outpatient Scheduling Experience required
• Certified in state(s) practicing as a Nurse’s Aide, or Certified Home Health Aide (CHHA), or Certified Geriatric Nurse’s Aide (CGNA).Can be a non-aide if the administrative team agrees on the candidate’s qualifications.
• Board Certified Hospice and Palliative Nursing Assistant (CHPNA).(Preferred)
• At least two years’ experience in Hospice care or Homecare with strong communication and organizational skills.
• Must have reliable automobile, valid driver’s license, and the minimum state required liability auto Insurance.
• A compassionate attitude toward the care of the sick; the ability to read, write and carry out directions, maturity and the ability to deal effectively with the demands of the job
• Minimum of one year data entry, word processing and/or medical records maintenance experience in a medical or healthcare customer service environment.
• Required Certification/Licensure:
• Preferred graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing
• Knowledge of Home Care Home Base EMR system
