Job summary

Moments Hospice is seeking a compassionate Certified Nursing Assistant (CNA) in Lincolnwood, IL to provide high-quality personal care and support to patients at the end of life. Duties include bathing, grooming, mobility assistance, and collaboration with a RN Case Manager to follow care plans.

Qualifications

• CNA license is required.
• Minimum 12 months caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice.
• Current CPR certification preferred.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care, etc.).
• Review patient care plans.
• Assist with lifting and transferring patients.
• Assist with range of motion, positioning, exercise, and ambulation.
• Assist with dining and nutrition.
• Maintain electronic medical records.
• Work under the direction of a Case Manager RN.

Qualifications:

• Certified Nursing Assistant (CNA) license (required)
