Job summary

Moments Hospice is seeking a compassionate CNA to provide direct patient care on the road. You will work under the supervision of an RN Case Manager, performing essential personal care, documentation in the EMR, and collaborative care planning.

The role includes a company car benefit and a robust benefits package. Qualified candidates have a valid CNA license, at least 1 year of caregiving experience in relevant settings, strong communication, and reliable transportation with a valid driver's

Qualifications

Must possess a valid CNA license.
Minimum 12 months caregiving experience in Home Care, Assisted Living/Memory Care, Skilled Nursing/Nursing Home, Hospital or Hospice.
Current CPR certification preferred.
Excellent customer service and strong communication skills (written and verbal).
Solid problem-solving and time-management abilities.
Ability to work in an EMR system and lift 50 lbs. with proper technique.
Valid driver's license and auto insurance required.

Responsibilities

Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care).
Review and follow patient care plans with RN Case Manager.
Assist with lifting and transferring patients.
Assist with range of motion, positioning, exercise and ambulation.
Assist with dining and nutrition.
Maintain accurate electronic medical records.
Work under the direction of a Case Manager RN.
