Job summary

Houston Transitional Care is seeking a Certified Nursing Assistant to provide routine daily nursing care and services to assigned residents in line with care plans and supervision.

Responsibilities include identifying residents, assisting with daily living activities, recording intake and vitals, maintaining cleanliness, and reporting changes in resident condition while protecting privacy and safety. The role requires teamwork and compassionate, respectful care.

Qualifications

• Must have at least a 10th grade education.
• Ability to read, analyze and interpret general business materials and regulations.
• Ability to write reports and documentation for resident records and care plans.
• Ability to present information clearly to managers and colleagues.

Responsibilities

• Provide routine daily nursing care and services per care plans.
• Identify residents before procedures and assist with daily living activities.
• Record entries accurately in flow sheets and notes.
• Report changes in resident condition to Nurse Supervisor/Charge Nurse promptly.
• Maintain confidentiality of protected health information and report disclosures.
• Assist in preparing residents for medical tests and transport as needed.
