Job summary

Pasadena Post Acute in Pasadena, TX, seeks a Certified Nursing Assistant to provide routine daily nursing care to residents per their care plans and assessments, under supervision.

You will identify residents, assist with meals, personal care, mobility, and documentation, report changes to the Nurse Supervisor, and maintain a safe, clean environment.

This role requires patient communication, teamwork, adherence to privacy laws, and accurate charting to support quality care.

Qualifications

• Must possess minimum 10th grade education.
• Licensed CNA in accordance with state laws.
• Ability to read, write, and communicate effectively.
• Basic math for measurements and documentation.
• Ability to interpret instructions from supervisors and follow care plans.
• CPR Preferred.

Responsibilities

• Identify residents using wristbands or photo IDs before care.
• Provide routine daily nursing care per care plans and assessments.
• Assist with meals, bathing, grooming, dressing, and mobility.
• Record entries accurately and timely in charts and flow sheets.
• Report changes in resident condition to Nurse Supervisor/Charge Nurse.
• Maintain confidentiality of resident information and ensure privacy.
• Support residents with daily activities and transportation as needed.
• Follow policies for exposure control and infection prevention.
• Assist in preparing residents for medical tests or examinations.
• Participate in nursing shift handoffs and quality improvement efforts.
