Job summary

St. Peters Post Acute is seeking a dedicated CNA to provide routine daily nursing care and services to residents in accordance with care plans. You will identify residents, document care, and report changes to Nurse Supervisors, while maintaining confidentiality and following facility policies.

The role involves assisting with activities of daily living, conducting basic health checks, and supporting a safe, warm environment for residents, with emphasis on resident comfort and dignity.

Qualifications

• Must be a licensed Certified Nursing Assistant in accordance with laws of the state.
• CPR preferred
• Ability to read, analyze, and interpret general procedures and instructions.

Responsibilities

• Provide routine daily nursing care and services to residents as per care plan.
• Identify residents before treatments and meals using wristbands or photo IDs.
• Record entries accurately on flow sheets, notes, and charts.
• Report changes in residents’ condition to supervisors promptly.
• Maintain confidentiality of resident information and report any unauthorized disclosures.
• Assist with activities of daily living and mobility under supervision.
• Observe and report any injuries or changes in health status to the Nurse Supervisor.
