Job summary

Optalis Healthcare seeks a Certified Nursing Assistant to deliver nursing and related services to residents in alignment with individualized care plans. The role supports residents’ physical, mental, and psychosocial needs, promoting independence and a homelike environment.

Responsibilities include assisting with ADLs, maintaining hygiene, documenting care, and communicating changes to the Unit Charge Nurse while adhering to infection control and confidentiality policies.

Qualifications

• High school diploma or GED required to understand patient care basics.
• Successful completion of a state-approved nurse aide program with competencies.
• Verification of training and registry compliance for abuse/neglect screening.

Responsibilities

• Perform basic patient care such as bathing, feeding, and ADLs; assist with procedures as directed.
• Make beds, distribute nourishments, and maintain a clean, orderly environment.
• Maintain personal/oral hygiene and support residents to preserve independence.
• Provide direct care per the resident's plan of care and document observations.
• Participate in care plan evaluation; report changes to Unit Charge Nurse and update records.
• Follow infection control policies and use PPE as indicated; observe universal precautions.
• Demonstrate responsiveness to changes and handle additional tasks as assigned.
• Maintain excellent interpersonal and customer service interactions.
