Responsibilities

• Provide daily personal care and ADL assistance per care plan.
• Maintain clean, neat resident rooms and care areas.
• Assist with bathing, dressing, grooming, toileting, feeding.
• Document care as instructed and report changes to nursing staff.
• Monitor vital signs and report changes.
• Assist with mobility, transfers, and transporting residents.
• Support residents in activity programs.
• Respond to call lights promptly.
