Job summary

Vitality Living Lake Forest in Sanford, FL is seeking a dedicated Caregiver/Personal Care Specialist to support residents with daily activities and emotional well-being, following individualized service plans.

You will participate in onboarding and ongoing trainings, report resident changes, and assist with plan development; must be at least 18, communicate in English, maintain required certifications, and meet health requirements.

Qualifications

• Must be at least 18 years old.
• Demonstrate ability to communicate effectively in English, both verbally and in writing.
• Maintain any certification as required by state regulations.
• Meet state health-related requirements.

Responsibilities

• Assist residents with activities of daily living, simple treatments, and emotional health needs as per service plans.
• Assist in development and revision of service plans by informing supervisor of resident needs and preferences.
• Observe and report resident problems to LPN or Medication Tech/Aid.
• Attend onboarding, annual and in-service trainings and team meetings as requested.
• Perform other duties as assigned by Wellness Director.

Caregiver/Personal Care Specialist Responsibilities:

• Assist residents with performance of activities, simple treatments, and emotional health and/or problem behavioral issues as indicated in service plans
• Assist in development and revision of service plans by informing supervisor of current resident needs and preferences, as well as successful service approaches
• Observe and report resident problems to LPN (if on duty) or Medication Tech/Aid
• Attend and participate in onboarding, annual and in-service training and team member meetings, as requested
• Perform other duties as assigned by Wellness Director
