Job summary

Vitality Living Lake Forest is seeking a Caregiver to join our Lake Forest community in Sanford, FL. You will assist residents with activities, simple treatments, and emotional health as outlined in service plans, and participate in onboarding and ongoing training.

Requirements include being at least 18, clear English communication, meeting state health requirements, and holding any state-required certifications.

Qualifications

• Must be at least 18 years old.
• English communication proficiency (verbal and written).
• Maintain required state certifications and health requirements.

Responsibilities

• Assist residents with activities, simple treatments, and emotional health and/or behavioral issues as indicated in service plans.
• Assist in development and revision of service plans by informing supervisor of resident needs and preferences.
• Observe and report resident problems to LPN or Medication Tech/Aid.
• Attend onboarding, annual and in-service training and team meetings, as requested.

Caregiver Responsibilities:

• Assist in development and revision of service plans by informing supervisor of current resident needs and preferences, as well as successful service approaches
• Observe and report resident problems to LPN (if on duty) or Medication Tech/Aid
• Attend and participate in onboarding, annual and in-service training and team member meetings, as requested
• Perform other duties as assigned by Wellness Director
