Job summary

Vitality Living Lake Forest is seeking a Caregiver/Personal Care Specialist to support residents with daily living activities, basic treatments, and emotional well-being. You will assist in developing and updating service plans, and report resident needs to the supervising team.

The role requires effective English communication, compliance with state health requirements, and participation in onboarding and ongoing training.

Qualifications

• Must be at least 18 years old.
• Demonstrate ability to communicate effectively in English, both verbally and in writing.
• Maintain any certification as required by state regulations.
• Meet state health-related requirements.

Responsibilities

• Assist residents with activities of daily living, simple treatments, and emotional health as indicated in service plans.
• Assist in development and revision of service plans by informing supervisor of current resident needs and preferences.
• Observe and report resident problems to the LPN or Medication Tech/Aid.
• Attend onboarding, annual and in-service training and team meetings.
• Perform other duties as assigned by Wellness Director.

• Observe and report resident problems to LPN (if on duty) or Medication Tech/Aid
• Attend and participate in onboarding, annual and in-service training and team member meetings, as requested
• Perform other duties as assigned by Wellness Director
