Job summary

Team Select Home Care is seeking a Certified Nursing Assistant (CNA) to provide skilled personal care and activities of daily living (ADL’s) to patients in their homes under RN supervision. You will report to the Registered Nurse and perform duties per state regulations and care plans.

Responsibilities

• Provide direct patient personal care and ADLs as outlined in care plan.
• Assist with medications as allowed by state law.
• Maintain infection control practices and proper hand hygiene.
• Document visits and report changes to RN promptly.
• Attend staff meetings and care conferences as scheduled.
• Ensure clean and safe home environment and equipment.
