Job summary

Team Select Home Care is seeking a Certified Nursing Assistant to deliver skilled personal care and activities of daily living in patients' homes under the supervision of a Registered Nurse. You will report to the RN and participate in care planning and documentation.

Responsibilities include assisting with ADLs, household tasks, and simple procedures, while maintaining infection control and timely reporting of patient changes. A valid CPR credential and state licensure compliance are required.

Qualifications

• High School Diploma or GED required.
• Must be 18 years of age or older.
• Must maintain licensure or certification per state regulation.
• Maintain current CPR and health certifications per state requirements.

Responsibilities

• Provide skilled personal care and ADLs under RN supervision.
• Follow care plans and agency policies.
• Assist with medications routinely self-administered per state regulation.
• Maintain infection control and hand hygiene.
• Document visits timely and accurately.
• Communicate changes to RN promptly.
• Attend staff meetings and in-service trainings.
• Complete visit records and reports.
