Job summary

Team Select Home Care is seeking a Certified Nursing Assistant (CNA) to provide skilled personal care and ADL support in patients' homes under RN supervision. You will report to the Registered Nurse and work within established agency policies.

Responsibilities include direct care, meal prep, documentation, and timely reporting of changes in patient condition. Requires HS diploma or GED, 18+ years, CPR/certifications, and flexible scheduling.

Qualifications

• Empathetic and compassionate attitude toward care.
• Ability to read, write, and follow directions.
• Effective communication with patients and RN supervision.
• Flexible and adaptable to patient needs.
• Ability to manage time and documentation accurately.

Responsibilities

• Provide direct personal care per care plan under RN guidance.
• Assist with ADLs, meals, and light household tasks in patient home.
• Document visit records promptly and accurately.
• Report changes in patient condition to RN in a timely manner.
• Maintain infection control and confidentiality at all times.
