Job summary

County of Weld is seeking a Case Management Agency Supervisor to oversee the CMA team and coordinate the Long-Term Care Waivers and State General Funds programs. The role is primarily based in the Greeley Office with statewide travel and occasional after-hours support.

The ideal candidate will demonstrate integrity, strong workload management, adaptability to change, and conflict-resolution skills to guide staff in delivering person-centered services to families and communities across Colorado.

Qualifications

• Bachelor's Degree or 5 years of LTSS experience as defined in the job posting.
• Experience in long-term care services and supports, aging or disability services, or related fields.
• Proven ability to supervise staff and ensure compliance with state and federal regulations.

Responsibilities

• Supervises CMA program staff, provides leadership, and ensures performance and compliance with rules and regulations.
• Maintains quality assurance for CMA programs through audits and client feedback; ensures timely reporting.
• Manages workflow, assigns work, and monitors deadlines to meet program goals.
• Acts as advocate for clients and partners with PASA and other providers to ensure access to services.
• Promotes teamwork, integrity, and confidentiality; follows policies and procedures.
