Responsibilities

• Participate in program training for HCBS waivers and other programs.
• Perform intake and initial screening to determine eligibility for long-term care programs.
• Develop and monitor person-centered care plans and coordinate services.
• Progressively increase case management knowledge and manage a caseload.
• Advocate for members' best interests and collaborate with partners.
• Maintain professionalism and teamwork across departments.
