Job summary

Rock County, WI seeks a Children’s Long Term Support (CLTS) Case Manager or SSC to manage caseloads of at-risk children, coordinate services, and document activities. This role emphasizes cross-system collaboration, advocacy, and flexible scheduling to serve families efficiently and safely within the county’s waiver programs.

The position requires a Bachelor’s degree in a related field, relevant experience, and certification readiness for CLTS Functional Screen.

Qualifications

• Bachelor’s degree in counseling, social work, psychology, or human services related field.
• Experience with children, youth, and families involved in child welfare, juvenile justice, mental health, physical disabilities, and/or developmental disabilities systems.
• Certified in the Children's Long Term Care Functional Screen (within two weeks of hire).
• Must possess and maintain a current unrestricted driver’s license and auto insurance.

Responsibilities

• Provide Case Management Services to CLTS-eligible children and CCOP participants as outlined (40%).
• Complete required documentation and maintain Functional/Financial eligibility using Avatar and related forms (30%).
• Collaborate with internal and external partners to assure coordinated care and health/safety needs (25%).
• Perform all other duties as assigned (5%).
