Rock County's Human Services Department seeks a qualified CLTS Case Manager or SSC to manage caseloads of at-risk children, coordinating services and supports to promote least restrictive placement and community-based solutions.

Responsibilities include functional screens, reviews, data documentation in Avatar, and advocacy with providers. The role requires travel within the county and a flexible schedule to meet client needs.

Qualifications

• Bachelor's degree in counseling, social work, psychology or related field.
• Experience with children, youth, and families in child welfare/mental health systems.
• Certified in CLTS Functional Screen within two weeks of hire.
• Valid driver's license and auto insurance per policy.

Responsabilités

• Provide case management services to CLTS-eligible children and CCOP-related cases.
• Complete functional screens, collateral contacts, six-month reviews and required face-to-face contacts.
• Document eligibility, services, and activities in Avatar and other systems.
• Collaborate with internal and external partners to ensure coordinated care and safety.
